Healthy Together West Willow Care Team

We have a collaborative, multi-disciplinary team, finding ways to close gaps and innovate with partners that bring unique skills to solve problems.

We partner with residents to lead, plan and deliver services that impact them. We draw on their strengths to meet needs and solve problems.

We work to promote the benefits of preventive care and healthy behaviors to facilitate life changes. We take a ‘whole-person’ approach to address physical health and social needs together. These factors are often closely related and influence health outcomes and wellbeing.

Grateful for the Help!

“I got behind in my rent. The Social Work Student Interns connected me with resources to work with my landlord to get me caught up on my payments. That was such a blessing for me.” — Anonymous
